Hours:
Monday through Thursday 8:00 am to Noon, and 1:00 to 4:30 pm.
On Mondays and Tuesdays, we take appointments.
On Wednesdays and Thursdays, we do our office updates.
Appointments:
Please call, email us or write to us to pre-schedule your appointment to make that time allotted for you.
We Produce:
- Certificates of Tribal Enrollment (CTE)
- Family Tree Charts (FTC)
- Certificate of Indian Blood (CIB)
- Form 4432 - Verification of Indian Preference for Employment in the Bureau of Indian Affairs and Indian Health Service, and
- Applications for Eagle Parts for Native American Religious Purposes.
We process Applications for Tribal Enrollment (ATE) and Tribal Identification Card Applications (TICA).
We print Tribal identification cards daily during our office hours.
For Enrolled Tribal Members living on the the Mole Lake Reservation, we have Wisconsin Sales and Use Tax Exemption Certificates.

Please call the Housing Department for a mailbox so that important information for you and your family members can be received by mail through the U.S. Postal Service.
